iSymphony does not respect Quick Create Extension settings

As per discussion on http://issues.freepbx.org/browse/FREEPBX-9636 iSymphony should respect the "Enable Usermanager" setting during Quick Create Extension in FreePBX 13. Currently, even if we set "Enable Usermanager" to "No", when opening the extension again for editing later the defaults for iSymphony user creation are set by default to "Yes" and require further input before the extension settings can be successfully submitted.

I realize that User Manager and iSymphony users are not exactly the same thing, but it's inconvenient/annoying to have to remember to set these back every time when you don't need a user created and takes away some of the handiness of Quick Create.
