Commercial Endpoint Manager set Snom user_pbxtype for idx 3 to 9

This is a followup to http://issues.freepbx.org/browse/FREEPBX-6705

Tested with EPM 2.11.0.1.82

The user_pbxtype value is set for idx 1 and 2 but not for the rest of the handsets 3 to 9.

e.g.

user_pbxtype idx="3"
user_pbxtype idx="4"
user_pbxtype idx="5"
user_pbxtype idx="6"
user_pbxtype idx="7"
user_pbxtype idx="8"
user_pbxtype idx="9"
