Broadcast campaigns hang up on recipients instead of following defined Destination

XACTDialer/Broadcast campaigns hang up on recipients as soon as they answer the call.  This seems to be sporadic as I can test a broadcast to my personal cell phone and occasionally the system will follow the defined Broadcast->Campaign->Destination although the majority of the time it just hangs up (9 times out of 10).  I have checked for updates, rebooted the system, and Stopped/Restarted the Broadcast service to no avail.  Attached is a sample of my latest test campaign which hung up on answer.
