Add a Subject Field to Send Fax UCP under Cover Page option, and send info in email

We have clients that use the subject field to keep track of their outbound faxes by placing case numbers in the subject field, which on the old system was included in the confirmation email. We would like the to add a Subject field for the Coverpage option on sending a fax, and that field added to the confirmation email sent to the sender.

It can also be included as a default field if it can be added to the confirmation email. That is the primary part we need, is a Subject field added for the confirmation email sent to the sender.
