Find/Follow-ME hangs up on converstaion - Third try.

I have had this issue for over a year on version 14 systems.  Now with latest updates version 13 system also hangup calls.  Call route:

DID number to announcement

Announcement to queue

Operator answers queue and manually transfers calls to an extension

Extension has follow-me active to ring desk phone and call cell phone.

Rings cell phone, if not answered before cell phone VM answers the call is disconnected.  Does not matter how long the timers are set for. 

Prior to last updates on system this phone system ran 350 phones with over 500 calls an hour for 5 years without this issue.  Not user changes other than updates for the last 4 months and now ALL follow-me calls hang up on users.  We have over 100 system all with the same issues.  Maybe we can get it fixed this time around.
