I would estimate around 3% of our calls suffer from this issue.
Calls come in to our queue and hear hold music. When an agent answers, they are usually able to get out a few words "Thanks for cal–" and then the call returns to playing hold music.
After placing calls all day long, I finally was able to duplicate the issue.
A 'core show channels verbose' showed my number in the queue:

It shows I am in the 'queue' app.
iSymphony doesn't show me in the queue.
'queue show 2100' shows the agents, but doesn't show my call in the queue.
