Voicemail Reports module half-way displays the first extension

When I go to the commercial module under Reports\Voicemail Reports, my first extension (x100 in my situation) is not listed at the top of the list. However, at the bottom of the page, some of the fields for x100 show up.

Here's the list of my top three extensions in order:
100 - Nicole Roberts
101 - Alan Biddle
102 - Phil Sandhagen

Here's what I see in Voicemail Reports:
101 - Alan Biddle
102 - Phil Sandhagen

File #1 - The list of my extensions. Notice how my first extension is 100.
File #2 - Top of the voicemail reports view. Notice how extension 100 is missing.
File #3 - Bottom of the voicemail reports page. Notice how the username and extension is missing, but the PIN number is in the name column. Some of the other columns are shifted to the left as well.
