When attempting to save a new recording, the browser returns a simple dialogue saying 'error'. A second attempt returns 'an error occurred trying to submit this document' from the browser. Shortly after, FreePBX states 'File does not exist File:/var/www/html/admin/libraries/BMO/Media.class.php:95'.

This happens when creating a new system recording. I was recording one from an extension on my system. The process goes fine during the recording of the original message over your phone, but fails when you hit the submit button, but only IF you have selected more than on option at the bottom (wav, g722, ulaw etc.). If you only select on from the list, it ends successfully. You can make your recording if you do them one type (wav, ulaw, g722 etc) at a time. You can even use the original recording from the extension, you don't have to re-record that part.
